Recognizing that early intervention
is a key factor in the provision of
timely, quality service, the
Department has been delegated the
authority to operate under an
open-referral system, which permits
the delivery of services to all
hospitalized patients, irrespective
of a physician, or other healthcare
team member referral or consult.
Within this context of operation,
the ability to engage in early case
finding is maximized, thereby
affording early comprehensive social
work intervention to those patients
at-risk. In this inner-city
environment, these typically include
the frail elderly, the chronically
ill population, the homeless,
neglected and abandoned infants,
trauma victims and the uninsured;
therefore, the screening process
utilized in the Department is
particularly designed to target high
volume, problem-prone, at-risk
patients.
Services
The Department's schema for the delivery
of in-patient services is a unit-based
system in which each nursing station has
an assigned social worker. Such a
deployment of staff supports the health
care team concept with respect to
collaboration and communication—two key
elements in providing comprehensive and
quality patient care. In addition to the
unit-based system, social work service
is delivered through the Emergency Care
Area and all of the Ambulatory Care
Clinics, particularly, the Mental Health
Clinic, the Cancer Center, the Family
Practice Clinic and the Chronic
Hemodialysis Center. With the exception
of the HUMED Center, social work
services are provided to all units of
the Hospital and all satellite centers.
