ARTICLE X. PROCEDURES FOR APPOINTMENT AND REAPPOINTMENT

Section 1. Application for Appointment

1. An applicant for appointment to the Medical Staff shall submit in writing an application on a form approved by the respective clinical department(s). The applicant shall be required to provide information concerning professional qualifications, health status, previous professional experience, claims history, and such other information as is reasonably required by these Bylaws and to comply with applicable Federal and local statues and regulations.
